I would like to take the physical fitness portion in school for my application for the scholarship. I was wondering what paperwork i would have to get and fill out with my gym teacher to send in. Where could i find this?

Go to to http://www.goarmy.com and find your way to the ROTC application. Log in, and this print out should be under “Need More Information?”
Or you could just ask the ROO for that school to send you a PT card, make sure that you get your APFT graded by someone who is credible (like an army officer). Unless your gym teacher was military, the assesions board probably wont even look at it.
