check your EOC (Evidence of Coverage), or your contract / policy.
Call the carrier (i.e. Blue Shield, Kaiser - the # will be on your ID Card)
Probably depends on if the surgery is deemed "Medically Necessary"

That would be a question to ask your insurance company. There is a discount medical program that would save you money on that procedure. Check the site below.

Most don't. You'll have to call the number on the back of your card and ask.
If it makes you feel better, weight loss surgery is NOT a permanent fix - studies show that five years after surgery, over 85% of people are at, or above, their pre-surgery weight. Because of that, and because of the risks involved, it's usually voluntary.
The ONLY long term weight loss/control that is effective, is lifestyle change - eating rabbit food and excercising regularly.

Make an appointment with a doctor who does this type of surgery. They will go thru your history and see if you have any reasons that the surgery would be deemed medically necessary. If you do they will pay for it and will OK it before the surgery. A friend of mine doesn't have any health problems and they denied paying for her surgery. Make an appointment and you will be able to find out. It takes about 4-5 weeks to get an answer from your insurance company
